DOG DAY CARE AGREEMENT

The Dog Club of Fort Collins (hereafter referred to DCFTC) and the owner of the dog to be admitted
into day care (hereafter referred to as Owner) hereby agree as follows:

1. Owner hereby certifies that to the best of his/her knowledge, the dog is in good health, has not
recently been exposed to any contagious canine illnesses and the dog’s vaccinations are current.
Owner also agrees to inform DCFTC if the dog becomes exposed to any contagious canine illnesses
prior to any future stays at day care.  In admitting the dog to day care at DCFTC, Owner understands
and agrees it officers, directors, employees and agents have relied upon Owner’s representations
that the dog is in good health and currently vaccinated and Owner’s representations regarding the
dog’s behavior history.

2. DCFTC will exercise reasonable care of the dog during its stay. Dog owner does acknowledge that
he/she understands that their dog will co-mingle with other dogs. In consideration of these services,
Owner expressly waives and relinquishes any claims against DCFTC, it officers, directors, employees
and agents, relating to services provided pursuant to this agreement, including damage or injury to
the Owner’s dog or damage or injury caused by the Owner’s dog, except those claims arising from
willful misconduct by DCFTC and further agrees to indemnify DCFTC against all such claims.

3. The Owner is aware of and understands the techniques used in the dog training programs offered
at DCFTC and if Owner chooses to have DCFTC train his/her dog, consents to the use of such
techniques on his/her dog and waives and relinquishes any claims against the DCFTC, it officers,
directors, employees and agents relating to any training provided pursuant to this agreement,
including damage or injury to the Owner’s dog or injury caused by the Owner’s dog, except those
claims arising from willful misconduct by DCDTC and further agrees to indemnify DCFTC against all
such claims.

4. Owner further understands and agrees that in the event of an emergency, DCFTC is authorized to
seek emergency veterinary care as DCFTC deems necessary and DCFTC is hereby released from all
liabilities related to the transportation, treatment and expense relating thereto. In the event of
death of my dog(s) while under the care of DCFTC, I authorize to have the body taken to a veterinarian for
storage, at my expense, to await disposition instructions from me.

5. Owner agrees to pick up his/her dog by 6:00p.m., and understands that Owner will be charged an
additional fee of $25 for any dog not picked up by 6:30p.m, and an additional $10 every half hour the
dog is left after hours. No dogs will be allowed to stay on the premises after closing. Owner will be
charged a minimum fee of $75 should the animal require overnight care. If dogs are not picked up
within 3 days of when Owner has agreed to pick them up and/or there is no contact from the owner,
the dog(s) shall be considered abandoned and DCFTC will dispose of the dog at the owner’s
expense. Owner further agrees to release, hold harmless and indemnify DCFTC for any claims
relating to the transportation and kenneling of the dog should Owner fail to pick up the dog prior to
closing.
